The Oaks Medical Practice

How Do I....
Obtain A Repeat Prescription?

Repeat prescriptions can be ordered in several ways:

  • By submitting the re-order form (the left side of your latest prescription) to the receptionist
  • By post or fax
  • Some chemists (Stuart Mouls, Ellacombe Pharmacy, Ideal Chemist Warmley, Shaunaks Chemist and Billings Chemist) operate a prescription collection service and will bring your request to us if you ask them to, and some of them will deliver your medication to your house.
  • Online by completing the form below.

If you wish to collect the prescription from us to take to the chemist, please allow 48 hours for us to get it ready. If you wish it to be sent to a chemist allow 72 hours before collecting your medication from the chemist. Please ensure you have written on the request the name of the chemist to whom you would like the prescription sent.

Please note that if you have passed the review date on your re-order form, the doctor will need to review and update your medication before issuing your prescription.
